Munson Home Health Care Sees 64% Reduction in Readmissions Using Vivify Health’s Remote Patient Monitoring Platform

As a result of this roll-out, Munson has already reported significant improvements, reducing readmissions from 12.8% to 4.5% in just 30 days for patients with COPD and CHF as primary diagnoses – a reduction of 64%. Other improvements during a 30-day timeframe include:

• Reducing their post 30-day readmission rate from 11.8% to 3%
• Achieving a 0% readmission rate for coronary artery bypass surgery patients
• Achieving better than 95% patient satisfaction
